*Greenlife Healthcare Staffing* is seeking a qualified and mission-driven *Licensed Master Social Worker (LMSW)* to join a respected review agency in *New Rochelle, NY*. In this dynamic position, you'll use your clinical experience and advocacy skills to investigate complaints and evaluate long-term care facilities across New York.

Key Responsibilities:
* Conduct complaint investigations and routine surveillance of long-term care facilities
* Review and evaluate practices related to resident rights, mental health services, care plans, and more
* Draft *Statements of Deficiencies (SOD)* using regulatory principles and within mandated timelines
* Verify correction plans and assess follow-up compliance
* Input findings into federal and state databases
* Participate in Immediate Jeopardy monitoring and testify in administrative hearings when required
* Follow Department of Health (DOH) policies and protocols
* Travel to on-site locations according to schedule
Qualifications:
* *Licensed Master Social Worker (LMSW)* – Active NYS licensure required
* *2+ years of clinical experience* in a medical setting (preferably a nursing home)
* Experience in *nursing home surveillance or regulatory investigations* (preferred)
* *SMQT certification* preferred—or ability to obtain within 12 months
* Strong attention to detail, documentation skills, and a commitment to public service
